• The primary purpose of our community support program is to provide treatment and restorative interventions to adults ages 18 and up. Our organization helps one individual at a time, to achieve and maintain their stability in the community by building their personal independence in the most appropriate, least restrictive environment. Community Support shall include goal-directed training in the following areas: • Daily living skills. • Appropriate behavior related to the individual’s health and safety. • Proper use of community resources. • Assessment of mental and/or physical health (i.e. encouraging proper nutrition, medication management and the like) By way of training and support, GVHC’s community-based service provides ongoing assistance to families. Our liaison activities and trusted partnerships provide linkages to local food pantries, clothing centers, holiday toy drives, school supplies and more. GVHC advocates for the needs of families to satisfy consumer goals. An individual’s primary diagnosis will indicate the focus of treatment. If an individual has co-occurring mental health and substance abuse disorders, the clinician will integrate treatment for both disorders and the clinician will bill for the primary presenting problem. This service is provided by a Licensed Professional (LP). We would like to know your fee, awards and grant resume.
